Admissions

Whether you are seeking short-term rehabilitation or long-term care, placing a loved one in a nursing facility can be an overwhelming and complex responsibility. Our highly trained and sensitive admission staff will be there for you and will guide you in admitting your family member.

If applying from a hospital, contact your Social Worker/Discharge Planner, who will arrange for a Patient Review Instrument (PRI) and Screen. The PRI and Screen forms are New York State required admission forms and provide most of the medical information we need to process an application for admission. You should be aware that hospital transfers often take place rapidly, so if you want a tour it should be arranged as soon as possible.

If you are applying from home please contact our Social Service/Admission staff who will guide you through our admissions process. If you are requesting a transfer from another nursing facility please contact the social worker, who will arrange for a PRI and Screen and facilitate the transfer with one of our admission staff.

We always recommend a tour when choosing a nursing facility. We are proud of our facility and will gladly arrange a tour for you. Please call the Admissions Department for more information. Weekend tours are also available by appointment.
